I know a host that fits all your requirements:

http://www.portland.co.uk/
15 MB (but only 100 MB transfer, of course)
CGI, PHP, MySQL (but they don't work 70% of the time)
FTP (it's almost always either down or full)
no-ads (this might be the only redeeming feature)
