I think that's the key part. So although the data might be stored in a picture it won't infect your computer if open an infected picture. You would still have to open the executable.Infected JPEGs are unable to replicate on non-infected machines - ie. machines without the extractor component installed (hooked in the Registry).

It looks like it changes registry hooks to change the associated file from whatever you had before to the virus. It's still an executable file, like all viruses. You run the initial executable, and you're infected.
