If you can FTP, you can pretty much do anything... there's nothing to stop you from creating a file that uses the extension .htaccess, then uploading and renaming it to .htaccess. That's what I did.

There are a number of similar hosting services like that too. sidez.net, f2g.net and f2o.org. Sidez is kind of limited I guess but it has four hosting plans. f2g.net has just 15 MB space and currently isn't accepting signups, but they allow you to edit your .htaccess file and also have PHP4...
1) They suck.
2) The server is down quite often actually.
3) The porn ads.
4) There are better services out there... try f2g.net, f2o.org or sidez.net.
There are a few other nice ones I know too:
http://www.burntmail.com - web-based only, but no ads ANYWHERE. May become paid service in the future but free for now.
http://www.gmx.net - POP3 and SMTP, but in German. 10MB, web-based access also available. I personally like this one best...
I'm looking for a free host that has
- No banner or pop-up ads (text is fine)
- At least 1 MB or so
- Bandwidth doesn't really matter, but should be able to handle up to about 200 visitors a day

Geesh. Geocities has Frontpage support. I'm not too sure about all their specifications though. And by Frontpage support I presume you mean Frontpage Server Extensions?
Try http://www.0catch.com or one of it's partners. 100 MB and 1MB file size max per upload. Subdomain and in English. Not sure about the bandwidth. If you add a <nobanner> tag in your page it will have a popup instead.
I used to use web1000 too, but I have switched hosts due to the problems they have. Web1000 updates the site based on the number of users accessing their FTP site, so if ftp.web1000.com takes some time, try using:
ftp2.web1000.com
ftp3.web1000.com
ftp4.web1000.com
ftp5.web1000.com...
